Team Leader (Southeast Emergency Service Program Fall River, Fall River)
Participate as a member of the clinical leadership team including the Program Director, the Assistant Program Director/CFT Manager and the Evening Clinical Supervisor who together collaborate to hire, train and oversee staff, student interns and support/peer personnel.
Oversee the overall treatment planning and implementation of the MCI intervention by coordinating the delivery of services to youth and families and the handoff of services to community providers within a 7 day period subsequent to the initial ESP response.
Responsible for ensuring that intensive case management, treatment planning, care coordination and risk assessment is followed through with each MCI intervention.
Complete assessments and treatment plans for children and adults who are in crisis in a comprehensive manner attending to their safety and to their clinical needs.
Assist the person in crises with engagement or reengagement in needed services, utilizing community based health, behavioral health, and social services that will ameliorate the presenting crisis.
Provide brief treatment and support until requisite community services can be accessed in order to minimize reliance on acute inpatient services.
Facilitate admission to acute inpatient care on an as needed basis.
Document all assessments and interventions in accordance with program and Bay Cove standards securing as needed financial and insurance related information to facilitate billing and payment for services rendered.
Sharing of administrative on call in rotation with Program Director, the Assistant Program Director/CFT Manager and the Evening Clinical Supervisor (24/7/365 coverage).
Perform all other duties and projects as assigned by the supervisor.
LICSW, LMHC, or other behavioral health category which qualifies as an Independently Licensed Practitioner of the Healing Arts is required.
At least three years of experience providing services to adults with mental health challenges is required.
At least one year of supervisory and management experience preferred Demonstrated ability to de-escalate crises in a manner that ensures safety for all involved and provides for the best care and welfare of the person in crisis.
Demonstrated knowledge of the evaluation process, including articulating the presenting problem, social, family, psychiatric, legal, and medical history.
Capacity to effectively assess mental status, formulate a plan, and develop and implement an appropriate disposition for all clients.
Demonstrated knowledge of varying social and behavioral health services and the nature of the treatment, rehabilitation, and support they offer.
Can determine the person's needs for services, refer to the appropriate resource(s), and facilitate the person’s engagement in these services.
Flexibility to work holidays, evenings and weekends on an as needed basis.
Professional writing and verbal communications skills.
High degree of personal organization, attention to detail, and time management skills.
Proficiency in word processing and other computer functions using Windows, Word, and Google Email and Apps.
CPR and First Aid certification required within 3 months of hire.
Ability to assess children and adults in their homes and apartments with stairs.
Use of personal cellular telephone for work communication.
Current State issued valid Driver’s License and use of reliable personal vehicle for work related travel Bilingual candidates are encouraged to apply, pay differential offered for candidates with fluency in certain essential languages.
