Job Summary TheCase Management Associate shall be responsible for:
Assumes the role of the manager while the manager is away or out of the office.
Assigns and monitors work of Case Management Associates.
Ensures scheduling and project coverage for Case Management Associates.
Leads new employee orientation and on-boarding for Case Management Associates.
Assists with training of new and existing CMAS on all department processes and provides feedback to management regarding their daily performance.
Maintains, reviews and creates all CMA related reference documents.
Ensures all Case Management Associates demonstrate competency in all aspects of the care management roles.
Serves as the key resource for internal and external contacts related to Case Management Associate roles.
Provides feedback to manager on employee performance evaluations.
In order to qualify you must have:
High School Diploma / GED, Bachelor's Degree preferred.
Minimum of 4 years Case Management Associate experience.
Demonstrates excellence in effective communication both orally and in writing and provide empathy in difficult interpersonal and professional situations.
Requires the analytical skills necessary to resolve problems requiring a professional level of knowledge in a specific field.
Functions as the knowledge expert on systems and technology that are routinely used by CMAs to document and support care management needs.
Leads efforts to resolve issues involving the most challenging situations with various levels of staff, patients, families and community organizations when other CMA colleagues are unable to resolve.
Advanced knowledge of MS Office Excel, Word and PowerPoint as well as navigating an electronic health record.
