This role offers a unique opportunity to lead one of California’s most innovative and mission-driven public health plans at a pivotal moment in healthcare history. As the only Medi-Cal health plan in California to fully integrate medical, dental, and behavioral health services, HPSM serves one in five residents of San Mateo County. The position is based in South San Francisco.

Health Plan of San Mateo (HPSM) is a local, non-profit Medi-Cal health plan founded in 1987. Governed by the San Mateo County Health Commission, HPSM has a mission to ensure that San Mateo County’s vulnerable and underserved residents have access to high-quality care, services and supports so they can live the healthiest lives possible. With more than 150,000 members enrolled across its health plans and programs, HPSM serves nearly all of the county’s Medi-Cal eligible residents. HPSM also sponsors special programs that help members navigate complex social issues, such as securing home-based health care and affordable housing. HPSM has a staff of over 400 employees, including seven executive leaders, and annual revenues of $1.1 billion.

Health Plans:
Medi-Cal: State and federally funded health insurance program for low-income individuals and families.
CareAdvantage: Health insurance program for people with both Medicare and Medi-Cal.
HealthWorx HMO: Coverage for In-Home Services and Support providers and eligible employees of the City of San Mateo.
San Mateo County ACE Program: Locally funded plan administered by HPSM for low-income adults who do not qualify for other health insurance.
Healthcare Programs:
HPSM’s five programs serve one in five San Mateo County residents, representing diverse, low-income individuals and families, seniors, people with disabilities, and children with complex medical conditions. These programs include:
- HPSM Dental: California’s first and only dental plan for Medi-Cal members that is fully integrated with medical and mental health care benefits.
- CalAIM: A statewide program by the California Department of Health Care Services (DHCS) that helps managed care plans like HPSM support high-risk, high-needs Medi-Cal members.
- HomeAdvantage: Provides eligible CareAdvantage members with routine and urgent home visits, 24/7 phone access to providers, and transition support after a hospital visit.
- Whole Child Model (WCM): A partnership between HPSM and San Mateo County to deliver coordinated care and services to eligible kids with complex medical conditions.
- Wider Circle: Helps older adults make new friends and develop healthy habits by attending fun social events in their community.
The 2024-2028 strategic plan has two areas of focus:
- Better health care experiences and outcomes for all members: Our primary motivation is achieving meaningful improvements for our members. This area of focus is the “true north” we will galvanize our efforts around in the years to come.
- Thriving organizational capacity and resilience: In order to meet the ambitious goals of our first area of focus, we know we must invest in the foundation of our organization in several specific ways.

About the Position
HPSM seeks an exceptional leader to guide this mission-driven organization at the forefront of community health. This is a rare opportunity to lead a high-performing public health plan with a powerful impact on the lives of thousands of residents, advancing health equity while shaping the future of healthcare delivery in San Mateo County.
The CEO will lead HPSM in aligning strategy, operations, and culture around two core priorities: improving healthcare experiences and outcomes for members and strengthening the organization’s long-term resilience and capacity. This will require the CEO to translate HPSM’s strategic goals into coordinated action across the organization including: improving health outcomes, expanding access to high-quality member-centered care, advancing health equity, strengthening an inclusive and high-performing culture, leveraging data and technology to drive better decision-making, and ensuring long-term organizational sustainability.
Success in the role will require the ability to align providers, county partners, community organizations, staff, and the Health Commission around shared priorities while fostering innovation, accountability, and operational excellence. This role offers a unique platform to influence health policy and advocate for vulnerable and underserved communities. The CEO will serve as HPSM’s public face, cultivating strong relationships with members, providers, community leaders and government partners at every level.

The CEO of HPSM will attract and inspire top talent, foster organizational excellence, and champion innovative solutions that connect healthcare delivery to broader community well-being. The ideal candidate brings a master’s degree in health administration, business administration, or a related field, along with at least 15 years of progressive healthcare experience managing complex programs. They will have a minimum of 10 years in management roles, including five years at the senior leadership level. Experience administering Medicaid, Medicare, and other public programs is essential, with direct experience in Medi-Cal preferred.
